Meal/Fee Waiver Application

  • The online meal/fee waiver application for school year 2022-2023, will be available July 1st at schoollunchapp.com or using the link below. Instructions on completing the application are listed below. Only one type of application is necessary to submit per household each school year (online or hardcopy.)

    Additionally, if your household receives a letter or email stating that your child has been automatically approved for benefits for school year 2022-2023, you do NOT need to complete an application. Please ensure that all of your children are listed on the letter, and notify the food service department immediately if they are not. Please call 847-755-6681 with any questions.

    A printable version of the 2022-2023 will be available mid-July.   Return completed application to High School District 211 Food Service Department, 1750 S. Roselle Road, Palatine, IL 60067-7336.  Follow directions carefully. Applications must be completed thoroughly or they will be returned or denied. Application hardcopies are also available at each school or can be mailed by contacting foodservice@d211.org.

    In accordance with Federal civil rights law and U.S. Department of Agriculture (USDA) civil rights regulations and policies, the USDA, its Agencies, offices, and employees, and institutions participating in or administering USDA programs are prohibited from discriminating based on race, color, national origin, sex, disability, age, or reprisal or retaliation for prior civil rights activity in any program or activity conducted or funded by USDA.

Additionally, program information may be made available in languages other than English.

    To file a program complaint of discrimination, complete the USDA Program Discrimination Complaint Form, (AD-3027) found online at https://www.ascr.usda.gov/how-file-program-discrimination-complaint, and at any USDA office, or write a letter addressed to USDA and provide in the letter all of the information requested in the form. To request a copy of the complaint form, call (866) 632-9992. Submit your completed form or letter to USDA by:

    Additional food assistance is available through the state of Illinois for students qualifying for free and reduced-price meals. District 211 students who are eligible for free and reduced-price meals this school year 20-21 or in school year 19-20 automatically qualify for Pandemic- Electronic Benefit Transfer (P-EBT) program benefits. If you have not reapplied for meal benefits and believe you may qualify please fill out an application with the district at https://adc.d211.org/Page/6669.
