Meal/Fee Waiver Application

The online meal/fee waiver application for school year 2024-2025 is now available using the link below or here. Only one type of application is necessary to submit per household each school year (online or hardcopy.)
Additionally, if your household receives a letter or email stating that your child has been automatically approved for benefits for school year 2024-2025, you do NOT need to complete an application. Please ensure that all of your children are listed on the letter, and notify the food service department immediately if they are not. Please call 847-755-6681 with any questions.

Return completed paper application to High School District 211 Food Service Department, 1750 S. Roselle Road, Palatine, IL 60067-7336. Follow directions carefully. Applications must be completed thoroughly or they will be returned or denied. Application hard copies are also available at each school or can be mailed by contacting foodservice@d211.org.

To file a program discrimination complaint, a Complainant should complete a Form AD-3027, USDA Program Discrimination Complaint Form which can be obtained online at: https://www.usda.gov/sites/default/files/documents/USDA-OASCR%20P-Complaint-Form-0508-0002-508-11-28-17Fax2Mail.pdf, from any USDA office, by calling (866) 632-9992, or by writing a letter addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
