Summer Camp Physicals for the Class of 2029

If your child will be participating in a summer camp, please make sure that a copy of your child's current physical (dated within the last 13 months) is on file with the Hoffman Estates High School nurse no later than May 30. Students without a current physical on file at Hoffman Estates High School will not be allowed to participate in their camps until it is verified by the nurse. Please note, HEHS will not be receiving physicals from the junior high schools.

Completed forms can be sent to HEHSnurse@d211.org.

Incoming Freshman – Class of 2029

In accordance with Illinois School Code, all students in the class of 2029 must provide a valid physical examination dated on or after August 11, 2024, and up-to-date immunizations to the Hoffman Estates High School Nurse’s office prior to the start of freshman year. Student physical exams dated after January 1, 2025 must be on the updated form (12/23) to be valid. Please note that we will not receive medical files from the junior high schools. Students will not be able to attend school until all physical and immunization requirements are met. Additionally, all incoming 9th graders must provide proof of a valid oral health examination prior to May 15, 2026. We recommend providing completed physicals as soon as possible to avoid any delays in starting school.

Completed physicals may be sent to HEHSnurse@d211.org. Questions about physical and immunization requirements can be directed to the Nurse’s office at (847) 755-5663.

Students in the Class of 2026

All rising seniors are required to show proof of receiving two meningococcal vaccines. This proof must be provided to and verified by the Nurse’s office prior to the start of the school year. Students who are not in compliance will not be able to attend school until this health requirement is met. We recommend providing proof of this vaccine as soon as possible to avoid any delays in starting school.

Proof of the vaccine may be sent to HEHSnurse@d211.org. Please contact the Nurse’s Office at 847-755-5663 with questions.
