5 Essentials Survey | Take the Survey Today!

The Illinois 5Essentials Survey  is a brief, 10-minute survey, and your answers matter. Your perspectives help guide improvement efforts in your child’s school. If at least 20 percent of parents at a school complete this survey, a supplemental parent report will be shared with us. We are confident we can reach that target with your support.

The survey assesses your perceptions of five key components of school success:

Effective leaders
Collaborative teachers
Involved families
Supportive environment
Ambitious instruction

The survey is online in English and Spanish at survey.5-essentials.org/Illinois

It is administered by UChicago Impact at the University of Chicago on behalf of the Illinois State Board of Education. Please call your school’s survey coordinator, listed below, with questions. Thank you for the role you play in making our schools the best they can be!
